Description

Thank you for your interest in the IIMS Community Engagement Small Project Grants Program. We are seeking proposals for one-year community engagement projects.  Our goal is to promote, develop, and expand community-academic partnerships for the translation of science from basic discovery to clinical practice to public health benefit. Applications are due January 14, 2019 at 5:00 pm.

INSTRUCTIONS

You will need to log in to Survey Monkey Apply in order to create an application. There have been some problems reported when using Internet Explorer and Edge browsers.  If possible, please use either Chrome or FireFox.

To download Chrome go to: https://www.google.com/chrome

To download Firefox go to: https://www.mozilla.org/en-US/firefox/new/

If you have cannot preview or download your application, turn off the pop-up blocker for your browser.

UT Health San Antonio Faculty or Students

Use your UTHSCSA credentials to log in to Survey Monkey Apply

Click the Go to site button below and then follow the instructions to log in or create an account.

Click the Log In link and select UT Health SA Login if you are a UT Health SA Faculty or Student and select SurveyMonkey Apply if you are not.

UT Health SA Faculty and Students will use your UTHSCSA credentials to login.  Others will need to create a SurveyMonkey Apply account and then use it each time you log into the system.

If you see this screen –

You will either click ‘Return to UT Health San Antonio’ (for UT Health SA employees) or create an account (all others)

Non-UT Health San Antonio Faculty or Students

Click the button below or go to https://apply-uthscsa.smapply.io to create an account and start an application.

Application requirements and format

When you begin the application process, you will see a number of tasks on the left side of your screen.  

Click on an open circle to begin a task.  The tasks do not have to be completed in order.  However, ALL tasks must be completed before you will be allowed to click the Review & Submit button.

An open circle indicates a task that has not been started.
A partially filled circle indicates an incomplete task.
A green circle indicates a completed task.

All supporting documents must be in PDF format before they can be uploaded.
